Care Coordinators Are the Key to Our Plan

As soon as you become a member of the Affordable Life Care Plan, a Care Coordinator will visit your home and establish a relationship with you. It’s the Care Coordinator’s job to understand all there is to know about your situation — your medical conditions and prescriptions, your personality and preferences, your living environment, and any special directives you may have. Should you experience a change in health, just pick up the phone and your Care Coordinator will arrange for, schedule, dispatch and supervise all the care covered under your plan. Our Care Coordinators act as trusted friends and advocates, anticipating our members’ needs, helping resolve their issues and monitoring their satisfaction. If you need help at any hour, our Care Coordinators are available around the clock, 365 days a year.
